Quick answer
If your permit is lost or stolen you must file a report with the police and then apply for a duplicate using the postal kit, attaching a copy of the report. The cost includes the revenue stamp, postal fees and the electronic permit cost.
Losing your residence permit does not mean losing your right to stay, but you must regularise your paperwork quickly: a missing document causes problems with employers, banks, at the Questura and during checks.
The procedure is called a duplicate (or “replacement” when the document is damaged) and is normally filed with the postal kit.

Step-by-step process

1. File the police report
Go to the Carabinieri or Polizia: the loss/theft report is the basis of your application.

2. Keep a copy of the report
You will need both the original and a photocopy to attach to the postal kit.

3. Complete the kit and mark it “duplicato”
State the reason (loss, theft or damage) and attach the required documents.

4. Pay and send
Revenue stamp, postal fees and the electronic permit cost. The issue contribution may not be due for a duplicate — check the amount requested by the office.

5. Wait for the appointment and collect the new permit
If your fingerprints were already taken, you will usually not need to repeat them; the Questura may still summon you.
